Prayer Ministry Schedule

Below is a list of the prayer ministry opportunities available at Christian Healing Ministries, Inc. Please contact the CHM office at (904) 765-3332 concerning any questions you might have or to make an appointment.

If you would like to schedule a 90-minute prayer appointment at CHM, please download the forms needed. Please complete all three forms (personal intake form, family tree, and occult sheet) and either fax (904-765-4224) or mail them to us at P.O. Box 9520, Jacksonville, FL 32208. We must receive the forms from you BEFORE we can schedule an appointment.

The forms are stored in a locked file cabinet and are kept CONFIDENTIAL. The forms are used to orient your prayer ministers to your current prayer needs before your arrival so that most of your prayer appointment is not taken up with preliminary fact gathering. If more than two years have passed since your last prayer appointment, you will need to fill the forms out again as we purge our prayer recipient files after two years of inactivity. We look forward to praying with you!

Prayer Ministry By Appointment (Daily)

Mondays, Wednesdays, Thursdays and Fridays
No personal appointments on Tuesdays due to open prayer sessions (see below)
Office hours are 9:00 AM - 4:30 PM
Individual prayer sessions with two trained volunteer prayer ministers
Appointments last 1 and 1/2 hours
Appointment required (Call office)

Tuesday Open Prayer Ministry (weekly)

Tuesday Afternoons: 1:30pm-3:30pm, please arrive at 1:30pm as it is first-come-first-served
Tuesday Evenings: 6pm-8pm, please arrive at 6pm as it is first-come-first-served
20-minute prayer time with trained volunteer prayer ministers
Soaking music while you wait, you may want to bring a bible, journal, pillow or blanket
No appointment necessary, but please call to confirm the event

October, 2011:
- 4th - NO open prayer
- 11th - NO open prayer
- 18th - (Generational Euch. at 6pm)
- 25th - NO open prayer

Generational Healing Service

A celebration of Holy Communion, which includes special prayer for generational healing
-Generational healing is freeing us from the effects of our ancestors
Forms needed: Family Tree Form
No appointment necessary, but please call to confirm the event
From 6:00 PM - 8:00 PM on these dates:
- October 18
- November 15
- December 13

Day of Healing Prayer

Individual prayer for healing in a group setting, which includes a Generational Healing Service
Forms needed: Family Tree Form
Appointment required (Call office)
From 9:30 AM - 3:30 PM on these dates:
- October 24
- November 21
